Key Requirements for the Form

Parent/Guardian & Physician Signatures: Both must sign the form.

Student & Medication Details: Student's name, grade, medication name, dosage, frequency, route, time and purpose for medication.

Physician Order: A doctor's order is needed for all medications, including OTC (over the counter).

Annual Renewal: Forms must be updated at the start of each school year or with any changes.
Medication Delivery & Storage

Adult Delivery: Parents/guardians must deliver all medication to the school; students cannot transport it.
